Description
Creating instructions ahead of time for the end-of-life care you want! Advance care planning is the process of communicating plans for future medical decisions. This process includes completing advance directives, written and verbal instructions, about your future medical care in the event you are unable to express your medical wishes. We will have staff and resources available to assist you in completing this important documentation, including notaries. You are encouraged to bring the person you trust to make decisions for you to the seminar.
